Initial Consultation
Be sure to bring any insurance card, as well as a copy of your sleep study for the initial consultation. Please arrive 15 minutes ahead of time. This is to accomplish all the necessary paperwork before the scheduled appointment. You may also request a form via email or fax to fill out prior to your visit. Additionally, our doctor can answer questions during lunch, from 1:00 p.m. to 2:00 p.m.
Referral Policy
If your insurance plan requires a referral, you are responsible for obtaining one. Be sure to contact your primary care physician prior to the appointment. For your convenience, they can fax the referral to our office. Securing a referral is essential for processing your claims.
Prescription Refills
Should you require premedication for dental treatment, please call the office at least 24 hours prior to your visit so that we may phone it in for you. Other refills will be evaluated on a case-to-case basis.
